Purpose: This form is used to report suspected violations of the Sin City Fire Department Sports Code of Conduct, organizational policies, or applicable laws.
Who Can Report: Any member, participant, board member, volunteer, or witness to conduct that violates organizational standards.
Confidentiality: Reports will be handled with discretion. Information will be shared only with individuals who need to know for investigation purposes.
Non-Retaliation: The organization prohibits retaliation against individuals who report violations in good faith. Retaliation is itself a violation subject to disciplinary action.
How to Submit:
Timeline: The board will acknowledge receipt within 3 business days and conduct an investigation within 14 days.
